SCHEDULE – Client agrees to 15 months of service. Homeowners are advised that children and pets should be away from the treated areas for 1-2 hours on the day of the service. American Pest Solutions will automatically renew this service unless otherwise notified in writing by the customer 30 days prior to the anniversary date of the plan startup shown above. American Pest Solutions reserves the right to adjust the fees payable pursuant to this agreement as needed. If you decide to cancel the service prior to the current anniversary date, there will be a $125.00 cancellation fee. American Pest Solutions must be allowed to remove any devices we have installed on the property. Failure to allow removal of devices will result in a $150.00 charge. Delinquent accounts over 90 days past due will incur a $75 reinstatement fee. This plan is 100% transferable to a new owner should the original party sell the property while the plan is in place.
ACCEPTANCE OF AGREEMENT – The above prices, specifications and conditions are satisfactory and are hereby accepted. You authorize American Pest Solutions to provide the service as specified. Service visits must be accepted as prescribed, no credits will be given. Payment will be made as outlined above. Coupons and or discounts must be presented on the date of signing and will not be honored past the execution date of this agreement. All material is warranted to be as specified. All work to be completed in compliance to all federal, state, and local regulations. Any alteration or deviation from the above coverage and specifications involving extra costs will be executed only upon written orders, and will become an extra charge over and above the job price quoted. All agreements contingent on strikes, accidents or delays beyond our control. Home owner to carry fire, tornado, and other necessary insurance. Our staff is fully covered by Workman’s Compensation Insurance.
